Do not worry, prepare the application materials on your own and directly mail them to the Bureau of Labor Insurance. In the section on the application form labeled “Insured unit certification column” detail the reason the company gave for refusing to stamp your application. The bureau will contact your employer for clarification and review the case based on the facts discovered.
